Last night i put my phone on the charger and when I unplugged it in the morning the battery symbol showed 100% but still showed the charging symbol and stated it was charging. When I tapped on the battery symbol is had the graph but stated "battery usage data isn't available". I shut down/re-started and it still shows "battery usage data isn't available". How can I get things back to normal.

You will need to drain your battery to zero and then recharge your phone. Sometimes that will fix the issue.

This sounds like a bad battery though and that isn't good.

I see that whenever I take it off of the charger after a full charge. It continues to display until the phone has been on long enough for it to initially plot a "battery time left" estimate.

I believe my Nexus 5 was hacked. Sudden drops from 100%-0% full to nothing happens when I tap on something. Could this be a bad battery, calibration, or an automated hack?
