Just talked to a VZW rep--she said there is an update--ICL53F, but I believe that is what we're at currently? No info regarding 4.0.4 or anything else, but they usually don't know much.

No. It should still push so long as you're on the stock 4.0.2, rooted be damned.

Honestly, I think it needs to be activated. Have no idea though, so hopefully someone with more knowledge chimes in.
