I'm having the same exact problem for the past few weeks now, and I've turned off Ok Google from any screen and as of today the issue has returned. I'm also on Tmo too.

Seems to happen everywhere, tardus. I think one call was better than the others but it was still hard to hear me according to the person in the other end.
@oregonbruin and @ragdoll08 - does this happen from a single location or everywhere? If a single location do some calls have "good" sound quality?
For me it seems to be everywhere. Today for an example, I called a friend of mine on bluetooth in the car and he said it was terrible quality. I restarted my phone and called him back, and he said I came in fine. After about 20 minutes the call dropped on his part. I called him back immediately and right away he said the voice problem was back and just as bad.
I've been noticing that the vast majority of issues callers had told me of were when I was on bluetooth over my car, however when I take them off bluetooth they said it didn't get any better. Even hanging up and calling back doesn't solve the problem, only when restarting the phone does it seem to fix it for the time being.
Color me clueless. How do I completely turn Google Voice off I the settings? I turned it off, I think, : language and input settings....google voice talking....disabled ok Google detection. Is that all?
