caller alert?

can someone tell me if they have this same problem?

i keep getting this popup everyday once i get an incoming call. i deactivate it from the settings but everyday it comes on. I've checked that i don't have any new apps. pls see attached screenshots.

there's an email to contact but i seriously don't think it's legit... help pls!!

I searched that email at the end of your screenshot and it looks like it is generated by this app -> https://play.google.com/store/apps/details?id=com.whosthat.callerid
