Double (Repeat) Apps in App Drawer

This is kinda weird. One of my apps (Speedtest) is showing 2 app icons in my app drawer. When tapping on the 1st one, it says permission denied (screenshot below) and when I tap on the 2nd, the app opens and functions just fine.

There's no options to remove the first app. So I uninstalled the functioning app and both icons went away. Then I was able to reinstall it from Play and everything seems just fine now.

No idea what would have caused that though. Anyone else seen something like this?

I had Magisk Manager do that - but both icons seemed to have run the same app. Same fix - uninstall then reinstall.
