HTC EVO 4G main screen shows nothing but the time, signal and charge?

My girlfriend tried to factory reset her(rooted) phone. The phone said "restarting phone," shut off, came back on an only the time, signal, and charge signs came up. The sd card is completely empty. What is wrong with the phone?

Sorry I can't post pics (no camera access). When I turn on the phone, the screen lights up and all. The only thing is that is shows nothing but the background and some icons at the very top.

In the pic, the black strip with the icons is all I can see. The rest of the screen is just the background (without the big clock, temperature, date, apps, etc).
