incoming calls going to voicemail when driving

all incoming calls go to voicemail when connected to car via bluetooth. DND is off. no issues when not connected to car

Welcome to Android Central! Is it possible the calls are actually ringing, but you're not seeing that since you don't hear nor see the phone ringing? And then the call goes to voicemail?

Make sure it doesn't later connect to another Bluetooth device thinking it's still connected to car Bluetooth?

No safe driving apps installed?

Try rebooting your device?
