SIM PIN and PUK error.

I wanted to put a lock on my SIM.
It's been asking for the PUK, and I found mine from My Verizon but it keeps saying the PUK is wrong. I enter the PUK, enter a 4 digit PIN and then it says the PUK is incorrect.

I'm sitting here with a useless phone now.

I fixed it.
I tried it once more, and it worked this time.
