Using the Prefixer app to remov

I have an unlocked Verzion phone on ATT. Some incoming calls that are actually in my contacts show up as the number instead of the contact. It's showing up as +1234567890 instead of (123) 456-7890. I read about and installed the Prefixer app and setup a rule to remove the =+ and it's not working. I tried the command \+ and \+[0-9]+ and neither actually remove the + sign. What's unique is it's not happening to every call, from a person in my contacts or otherwise.
