Android 6 Marshmallow will not let me use Google Voice to make outbound calls

Since updating to Android 6, I cannot use Google Voice to make any outgoing calls

- Nexus 5
- used GV to make all calls for more than a year without issue
- Incoming calls still work and go to GV voice mail
- Since Marshmallow update all calls show physical phone number instead of GV number
- Have tried switching to "Ask everything I make a call" and it does not ask
- Have deleted GV and reinstalled GV
- Have restarted phone and cleared caches several times
- I tried deleting Google Voice v0.4.7.7 and installed an APK for v0.4.6.1.

Even after installing an older version which had previously worked on my phone, Google Voice will not allow me to dial out. That leads me to believe it is something in Android 6 Marshmallow

Nothing I have tried works. Every time I call, the phone ignores my settings and uses the standard dialer to make outgoing calls instead of using my GV number. Can anyone help me?
