Battery "Dies" In 1 Hour, Phone Turns Off When I Plug It In, Shows "0%" When Plugged In

My phone has been fine for a long time. I have a Samsung Galaxy S3 and rooted it about a year ago. Just this weekend I changed chargers and suddenly, tons of problems. It may just be a coincidence, but I'm trying a new charger this afternoon. Assuming that isn't the problem (which I think is valid at this point), here are the symptoms:
-Refuses to charge
-When I finally get it to charge, the battery dies in literally about 45 minutes. From 100% to turning off of its own accord.
-I plugged it into my computer at about 16% battery and it almost immediately turned off. When I booted it while plugged in, it read "0%" battery. Then, I navigated to the files to try to get the logs out, but it died, while plugged in, and turned off.
Please ask questions. I'm running stock KitKat, just rooted.
