How can I make a phone as simple as possible?

My 80 year old grandma has one of those government phones. A while back, they switched out her flip phone with an android phone, and it's been nothing but trouble since. She does not understand the concept of a smartphone or apps or a recent calls list AT ALL. She can never find her contacts or the number dialer and is constantly accidentally opening apps and not knowing how to get rid of them. Is there any way at all to make it so the phone just displays nothing at all other than the dialing pad or something, to make this thing easier for her to use? I tried removing every app from the home screen, but even that has not made it easier for her to navigate.

Welcome to Android Central! Installing a custom launcher like BIG Launcher is an option.

Also, which phone does she have? Some manufacturers include an "easy launcher" version of the homescreen interface that simplifies things, like Samsung's Easy Mode.
