Exporting memos and s planner from samsung to another phone?

I am buying a new phone. I want to export all my memos and s planner (both built-in apps) from samsung galaxy grand prime to my pc and then to my new phone. How I can I do that? Please brief me. Thanks in advance.

You can try using Samsung Smart Switch from the Play Store of the PC version. The PC version may give you more options to back up over the app.
