For the past couple weeks my phone would randomly freeze and I would have to restart it (2-10 times a day). The other morning after having charged all night, it was only at 5%. A couple blurry lines went across the screen and it just shut off. After being on the charger for nearly an hour I finally got it to turn on again, but today and yesterday even charging for 8+ hours, it is barely getting to 30% and it dies not soon after I have to unplug it. A little while ago the same thing happened with the blurry lines and now it won't turn on. I have to get up early please help.

Welcome to Android Central! The battery might be failing. The phone is almost 3 years old, isn't it?
