Probably the single feature I liked best during my brief stint with a Samsung Focus was their camera button.

Anyone know of a good way to simulate this via an app or rom? Maybe double-tap volume button or something?

I really hate trying to make sure my finger is gonna press more or less where the shutter is when I'm reaching for a shot without it in front of me...

Not a solution, but a workaround: use the timer function.

you could always use the front facing camera for shots where you would normally turn the phone around
