Step 3 in the process causes the new SIM card to be recoded to the unlimited line. That is why it is safe to do the steps that I outlined. As long as you do not activate the new phone on the dumb line, it will stay at $9.99 a month. Basically, when the phone arrives, do not turn it on. Instead, immediately log into Verizon and activate it on to your wife's unlimited line. The IMEI and SIM numbers should be easily available (usually a sticker on the box).
With this said, if you feel more comfortable getting a SIM ahead of time, the steps still work. You'll just use the SIM you got rather than the one with the phone. Either way, you'll end up doing the same thing...logging into Verizon, coding the SIM for your wife's unlimited plan, and activating the phone on her line.
As for the upgrade fee, there is no way to avoid it.
