Why are you looking through your bf's history? We discourage violating other people's privacy, even if it's a significant other. I'm sure you wouldn't want him looking through your phone without your permission.

We get this a lot , it's your phone app and google activity monitors all activities on the phone, nothing to be alarmed about.

I know my boyfriend keeps hacking my phone
It won't let me access certain apps or look at certain websites there are files I know r there but it won't let me open or access and everytime I get close to figuring out what it is he does something else if I factory reset it comes back everytime and thw only thing I can find out is something about forum and c++ was on there dont know if that helps but I'm going crazy I don't know what he is trying to hide and why would someone have incallu on their phone whats the purpose cause I can't search to find out PLEASE PLEASE PLEASE HELP ME

All you need to do if you think someone hacking which is rare these days especially from a boyfriend is change all passwords/pins/ and require 2 step authentication to your accounts , pretty much all is needed .

If you truly believe he's hacking your phone and can prove it you should contact your local authorities as hacking is a crime.
Well, that's the system app that is the interface you see when in a phone call. Nothing to hide there, ANYONE that makes any phone call on certain Android devices will see that in their usage history (my Pixel 2 XL doesn't seem to have this, it just shows the phone app).
If your allegations are true, hopefully he earns the "ex" soon, that's no relationship anyone deserves to be in.

If you don't have a screen lock set (with a PIN or password that are hard to guess), and if you let other people have access to your phone, then you leave yourself wide open to various shenanigans. If you do have a good screen lock set up and never let other people set up your phone, then either your BF is Elliot Alderson (Mr. Robot, go watch the show if you haven't, a masterwork) or there's some other non-sinister explanation for the buggy behavior.
I wish there were some way for Google and phone manufacturers to label system apps in a way that would calm people down, like some footnote saying "THIS IS NOT MALWARE." InCallUI is the User Interface while you're In a Call.

He's a narcissistic control freak, and the sooner you get rid of him, the better off you'll be. Normal people don't distrust their mates, whether married, living together or just dating. If he doesn't trust you, he shouldn't stay with you. If he trusts you, he shouldn't be playing with your phone like that.
There's nothing technical you can do, except learn enough programming that you're better than he is, and keep him out of your phone (and you'd have to invest a few years into doing that - which hardly seems worth it to me).
The only help you need is this:
