I disabled voice wake up in S Voice and now "OK Google" works fine.

Had both active on my last phone, I guess now it's one or the other. Guess I'll stick with Google only since S Voice is not activating the mic when it opens.

This worked for me too, thanks for that. I'll have to wait to see if it continues to work. I'm hoping this might have fixed my voice "answer" feature also.

Yes I too have problems with this I have the switch turned on so I can say ok Google from any screen and I still have problems I just bought this brand new LG G for virgin mobile as well as my lg volt I just got rid of even my samsung galaxy tablet s2 8 inch also having problems it's definitely Google it's not our devices that much money in a company and they can't get something simple like this to work I don't know but I be firing somebody.
 
I know the last post was 2+ months old, but I had the same problem and found a fix by doing the following and hopefully it will help someone. when I had my eyes on the Note 4, one of the things that really intrigued me was able to wake up the phone while screen is off by saying 'ok google' and I was so bummed that it didn't work. Then I stumbled upon this thread and gave me some ideas to try and here's what I did to get it working:

1. go to settings/application manager and stop svoice
2. go to google settings/search & now/voice/'ok google' detection and train or retrain your voice model

that's it, it should work now. I even confirmed that restarting my phone 'ok google' with screen off still works.

Since day 1 I have been unable to get Ok Google to work properly. I have Ok Google Detection all turned on, trained my voice at least 15x (I speak English as a first language) and have tried just about everything yet it doesn't work. I can say "Ok Google" until I'm blue in the face and my phone does absolutely nothing.

Any ideas???

EDIT: I googled the problem and people are saying that Ok Google only works when I'm in the Google Now screen or am using Google Launcher. That doesn't quite make sense though- why would the 'always on' and 'lock screen' options be available then?

FIX SOLUTION...Just turn off all accessiblity..in android setting go to accessiblity then turn off all accessiblity app option then go to goigl app and u now able to retrain voice in Google app.some of app try to disable mic like kingoroot with the help of accessiblity.just turn it off...do not say thanks to me...just notify me.
