Good thinking. I was on an extender until about a month ago (got a great new router and don't need the extension any longer). I'm also on a WISP which can lose it's connection often as well as the cell signal is so weak my battery drains quickly. So, considering everything, I'm happy it works as well as it does.You said you wake up and have to use your fingerprint, which reminded me of a similar problem I had with it. I don't know if this applies to you, but we are using a Network Extender in our home since we do not have a good cellular signal. If our Network Extender loses its GPS signal, it doesn't know where we are and I found that secure locations did not then. If you are on the Network Extender, when you flick on your screen, the lock screen will say Network Extender in the upper left corner.
Another thing to check is in Settings under Location, make sure location method is set on high accuracy.

It's a comfort to know it always locks when away from home.
I've just got this problem with my Nexus 5 on Android 5.0.1.
It was working fine until a few days ago when it just stopped working without me doing anything to the phone.
I also get the Google Play Services error pop-up when I click on my home address in Trusted Places, and I don't see a way of deleting it only editing it? Also the Uninstall Updates button is greyed out in Google Play Services, so I can't do that either. The lock button on the home screen still has a pulsating ring around it so that can't be a problem either.
It's so frustrating when something stops working for no discernible reason.

I think I've solved the mystery of why some mornings I'm asked to put in my pin. I now have a Nexus 6P. After a week of use, it requested the pin as opposed to being Smart unlocked, it said something to the effect of: Whenever the device has been unmoved for 4 hours or more, you will be requested to enter in your pin. I have elderly dogs who rarely go 4+ hours without needing to go outside. Sadly, I now know that I rarely sleep more than 4 hours consecutively.Smart Lock has been working again for me ever since I put in several neighbors addresses...most of the time. About once a month I wake up to a request for my lock. Works perfectly in my car though.
I found a 100% solution (Note 4 with Gear S)
It's all about device administrator apps.
I had exchange client got there and then it got blocked.
Once removed works fine on spot.

Just wanted to let everyone know I tried all the methods in the thread but was not able to get trusted locations working on my Lenovo Phab 2 Pro. I gave up and accidentally fixed my issue. I powered off my Phab 2 Pro and moved my SIM card to another phone to test it out for a few minutes and needed info off my Phab 2 Pro, so I booted it up without the SIM card and noticed the trusted locations was working again. I powered off the Phab 2Pro, put the SIM card back in, booted it back up and location services are still working correctly again.
Just thought I'd share for anyone having the same issue and the previous methods didn't fix it.
