I was stock, rooted, unlocked, and the OTA just would not install.
Took me forever, had to revert back to a previous version altogether, and then clock work the OTA zip update manually.
After that, I attempted restore apps and SD from toolkit backup, which worked but apps were so messed up, and not all my SD files even came back.
So once I had jelly bean, I just did a factory reset, and started over with fresh app installs.
Finally after many hours of frustration, jelly belly is great.
I don't understand why the OTA just didn't overwrite, u wasn't even on a custom rom, just rooted and unlocked.
Oh well, with the overwhelming support by xda, I got all the files I needed to correct it.
So now I'm stock jelly, rooted and unlocked, and buttering my bread
